If your username and password doesn't log you in, you could try
Resetting Your Password.
By doing so, you will confirm you are registered with DECIPHER, using the correct email, and that your password is now what you believe it to be.

Did you know you can contact the staff responsible for or related to an individual patient directly?
If you go to that patients page, there is a contact tab on the right hand side, which will allow you to send a message to the responsible clinician and / or responsible staff.
